Requirements for New Passport

1.     Fee @ PHP2,100.00 (Subject to change without prior notice)

2.     Application Form

3.     Birth certificate from National Statistics Office

4.     Marriage certificate from national Statistics Office (for married women only)

5.     Photograph (3 copies) size: 4.5cm x 3.5cm

a.     Studio taken

b.     Color white background

c.     Dress or shirt should be with collar and sleeves (male or female)

d.     No eyeglasses

6.     In case of late registrations

a.     Submit the following documents with date and place of birth (e.g. school records, voters ID/voters addifavit, SSS ID, NBI Clearance, Community Tax Certificate, etc.)

7.     In case of minor applicants:

a.     Personal appearance of either parents

b.     Passport of parents

c.     DSWD clearance if not travelling with parent

8.     Personal appearance/bring at least 2 identification card with photo

9.     Others (Additional documents may be required by DFA)

5.     For Brown Passport (Issued prior to May 1, 1995) to submit documents with full middle name/need personal appearance

6.     In case of minor applicants:

a.     Personal appearance of either parents

b.     Passport of parents

c.     DSWD clearance if not travelling with parent

7.     Personal appearance/bring at least 2 identification card with photo

8.     Others (Additional documents may be required by DFA)
